• Completed DS-11 Form in BLACK INK
• Certified Copy of Birth Certificate
• Photography Services On-Site - $14.99 - Debit/Credit Card ONLY
• Original and Copy of front/back of applicant/parent(‘s) driver’s license

MINOR APPLICANTS (UNDER 16)

• Minor MUST be present
• Both parents MUST be present OR bring completed DS-3053 form (Notarized Statement of Consent) AND front and back copy of Identification from absent parent

Everyone must apply in person.
