- Completed DS-11 Form in BLACK INK ONLY (applicant must be present)
- Certified Copy of Birth Certificate and photo copy (long form with parents names) or Naturalization
- Photo Booth Self-Service Available On-Site - $14.99 - Debit/Credit Card ONLY
- (NOT RECOMMENED FOR ANYONE UNDER 4.5 FT TALL or WHEELCHAIR BOUND)
- Photo Identification Original and Copy of front/back of applicant/parent(s)
MINOR APPLICANTS (UNDER 16)
- Minor applicants must be present
- Both parents must be present unless one parent has a notarized Statement of Consent from the absent parent with copy of Identification front/back

- $22.05 - Additional 1-2 Day Delivery for Passport Book Only
