Forms are available in the front lobby of the courthouse. Once the form is complete you may come in the office to have your request processed. The Probate Court has the capability of certifying most Georgia birth and death certificates. When requesting a certified copy of a birth certificate or death certificate, a valid photo ID is required.
The cost is $25.00 for the first certified copy and $5.00 for any additional per request. If requesting for someone else, only certain individuals can obtain the birth or death certificate proof will need to be provided to determine that you are able to receive the birth or death certificate. If you are a grandparent requesting a birth certificate for your grandchild you must supply a copy of your child's birth certificate.
Birth certificates for Fayette County home births are processed through the Probate Court. Packets are available for pickup.
